SUBMIT A TIP: Incident Reporting System

Sandy Hook Promise
Developed in response to the Sandy Hook tragedy, this evidence-based anonymous reporting system is designed to help prevent violence, self-harm, and other concerning behaviors in schools. Students can confidentially submit tips or concerns related to bullying, self-harm, harassment, physical abuse, threat to school or students, and vandalism through this secured website. All reports go directly to a team of professionals at a crisis management center. Once a tip is received, an investigation is started, and school administrators and counselors are informed. In the case of an imminent threat, law enforcement will be notified.

*St. Serra Catholic School and its staff are not responsible for addressing reports of self-harm or potential threats submitted via the Sandy Hook Promise app or any other form of communication after 4:00 p.m. on school days, or at any time during weekends or school holidays.

This form will be monitored during regular school hours. If the report is an emergency, individuals should call 911 immediately.
Please note the importance of this reporting form and be aware that law enforcement may be involved in the monitoring of tips received.
