NEW Check Request Form now available!
A new, interactive check request form is now available to complete and submit.
To utilize the form:
· Download the form to your computer
· Open the form
· At the top of the form a yellow box will appear which states "This document contains interactive form fields." (if you have Acrobat Reader > v.7, your fields may automatically highlighted)
· Check the box on the right to "Highlight fields"
· Each of the fillable fields is now highlighted
· Click on the first field (Name or Organization Requesting Check)
· Type in the appropriate information
· Hit tab to jump between fields
· Print
· Submit with documentation (receipts) to the Business Office
